A Guide for Patients

Australian Clinical labs Functional Pathology

If you are new to using Functional Pathology testing then the information in this guide is for you. We hope it answers some of the more commonly asked questions we hear from patients new to using functional pathology testing services.

1. Practitioner Request Form

You will need a Practitioner Request Form from your health care practitioner. Your practitioner will select your test/s and sign this form. Note: This form must be returned with your completed Collection Kit.

2. Order a Collection Kit

Once your practitioner has given you a completed request form, please order a Collection Kit by calling Customer Service on 1300 55 44 80 between the hours of 9.00am and 5.00pm weekdays. All of our Collection Kits are prepaid when ordered. Payment can be made by Visa or Mastercard, cheque or money order.

3. Blood Collection

If your test requires blood collection, take your completed request form, to your nearest Australian Clinical Labs Collection Centre. Please call 1300 41 89 55 or visit www.clinicallabs.com.au/location/ for a full list of Australian Clinical Labs Collection Centre locations around Australia.

4. Returning Specimen

sSpecimens are returned via post or courier. A reply paid envelope is provided in the Collection Kit for all tests returned by post. If you are performing one of our tests that requires return via courier, a consignment note will be supplied in your Collection Kit. Call the courier company, Toll Priority on 13 15 31 the day before you require a pick up. Book a pick up for a Monday, Tuesday or Wednesday such that your specimens are returned to the laboratory by Friday.

5. Storage of Specimens

If you are collecting more than one specimen and over a number of days, store your specimens according to the Collection Kit instructions. When all collections are complete, return all samples to Clinical Labs Functional Pathology.Thank you for taking the time to read this guide.
